Speaker: Professor Eimear Muir-Cochrane, BSc Hons RN (University of London), Grad Dip Adult Ed. (UniSa), PhD (RMIT) FACMHN, Credentialed Mental Health Nurse.
Professor Eimear Muir-Cochrane is Chair of Nursing (Mental Health at Flinders University), and visiting professor at City University, London. Eimear is an Associate Editor of the Journal BMC Psychiatry (Social Psychiatry).
She undertook a BSc Hons in Nursing at the University of London, then her mental health nurse training at the Maudsley and Bethlem Hospitals in London. Eimear has published multiple books, chapters, monographs and papers in mental health since the early 1980s with funded research on seclusion, aggression and violence, physical restraint, youth homelessness and absconding. She is passionate about making a difference to patients experiences of psychiatric hospitalization, to reduce the use of restrictive practices used on patients and to contribute to nursing knowledge in mental health.    Seminar Abstract: Attempts to eliminate coercive measures in acute psychiatric in-patient units have resulted in a reduction of seclusion across Australia. However, the use of chemical restraint has received less attention. This paper reports on the incidence and characteristics of chemical restraint events occurring in Adelaide, using a descriptive quantitative design. Data were collected involving all reported chemical restraints occurring to adult consumers over a one-year period on 12 adult acute inpatient psychiatric wards in Adelaide, South Australia.
There were 166 chemical restraint events involving 110 patients. Out of 12 acute inpatient psychiatric wards, approximately 77.1% of chemical restraint events occurred on two closed wards. The highest prevalence rate between these wards was 28.78 chemical restraint per 1000 occupied bed days, compared to the lowest one being 0.12. More males (n=69, 57.5%) were involved in chemical restraint. Very few events reported the use of de-escalation skills in documentation. The two most common drugs were olanzapine and clonazepam. The use of chemical restraint was predominately combined with guard standby (74.1%). Findings demonstrate there are specific characteristics of patients more likely to be chemically restrained than others. Further, that chemical restraint occurs early in admission, is more likely at certain times of the day and is commonly associated with the presence of security guards.
Implications for practice include a need for systematic data collection of a range of variables as well as sustained focus on reducing the need to use chemical restraint.

E Mental Health Research An Introduction

Stig Helweg-Jørgensen, a PhD student in the E Mental Health Research Unit, introduces the mDiary study, one of the ENTER suite of studies funded by the Innovation Fund, Denmark. 
By listening to this podcast you will learn about: 
* The development and use of app based self monitoring and clinical monitoring collection of information on a cell phone aligned with the principles of Dialectical Behaviorual Therapy for people with suicidal and self harms thoughts and behaviours. 
* Piloting the app so far reveals a preference for cell phone rather than paper-based approaches for registering diary information by participants.
*The mDiary app is a useful and convenient way to monitor emotional oscillations and provide time sensitive feedback to users and their clinicians.
* The mDiary app provides a helpful platform for teaching skills in emotional regulation. 
Stig is using a time series analytical approach to investigate the data he has collected so far, but early signs are showing some very pleasing results about the acceptability of an app based diary approach to managing the suicidal and self-harm thoughts and behaviours for people with borderline personality disorder. The full results of this study are likely to assist people to take a greater role in managing their emotional regulation with others and help to provide meaningful feedback in the clinical setting as well. Abstract 
Human Computer Interaction (HCI) implications for the safe use of digital therapies targeting people with depression and/or anxiety
Despite the proliferation of digital mental health resources and treatments, the description of human computer interactions (HCI) in the international literature regarding digital therapies for people with depression and anxiety disorders is scant. Health professionals are increasingly interested to develop and administer digital health interventions for clinical treatment plans and pathways. Proponents of digital health highlight implied therapeutic and economic benefits to promote inclusion of these types of interventions. Yet, little attention has been given to the impact that design elements and HCI are likely to have on patient safety, effectiveness and with adherence to treatment. This represents a significant gap in the literature that may be a risk for the future safe and effective development of innovative digital health interventions. An integrative literature review was conducted to investigate how adequately HCI is incorporated in the development of E Mental Health interventions for depression and anxiety. The PRISMA model was used to discover, select and include relevant articles from suitable databases. The main outcome of this review is that HCI aspects are superficially described in literature about digital health interventions, but insufficiently to scientifically validate design principles and HCI. This is an important limitation in most literature because it compromises the innovation and leaves those who manage and implement the interventions, such as mental health professionals, with incomplete evidence to support the safe, reliable, and evidence-driven implementation of the interventions. Research implication: Digital Health researchers should consider the need to include computing science and HCI principles as a core element of research design to improve the quality and safety of interventions they trial or implement in the future. Mental health clinician implications: Mental health clinicians should assess and consider the safety of incorporating digital health interventions based on sound and robust evidence.
