His understanding is different than Donnie's, everyone forgets Frank was alive and real up to the point Donnie kills him. Nobody dies until the "end" of the film when the living Frank runs over living Gretchen, Donnie shoots Frank, and Donnie retreats upon the hill to accept his own fate. Each character's experience of "time" and consequential death are all relative to the inconsistent world around them.
