The first two days of November 2018 saw over 110 MS Health Care Professionals (HCPs) gather to discuss how we can end the variation in MS services across the UK.

HCPs, service providers, charities and planners all came together to hear eminent individuals talk about topics ranging from policy and data, to patient perspective and empowerment. The room then collectively questioned, discussed or debated that topic in relation to MS services, looking to decrease variation and optimise services for those living with the neurological condition.

Variance in MS Services, for example, access to and the prescribing of disease-modifying therapies, have been well documented across England by surveys performed by the MS Society, MS Trust and the RCP. This has been backed up by data presented at the ABN in Birmingham from the NHSE Bluteq database. We and others feel this is a problem for people living with MS and is a barrier to improving outcomes for pwMS being managed by the NHS in England. To address this problem we held this initial meeting of representatives from MS DMT prescribing centres across England. The aim of the meeting is to generate ideas and proposals to address and solve this problem. We envisage holding subsequent follow-on meetings to implement and test potential proposals developed as a result of this meeting.

MS Variance – Dr Geraint Fuller: GIRFT

Dr Camille Carroll, University of Plymouth, introduced the audience to the Clinical Research Network and how can it assist researchers.

For a number of years we have received feedback at the Parkinson’s Academy about including sessions about Research. Taking these responses and working with The Cure Parkinson’s Trust (CPT) and NIHR leads Dr Camille Carroll and Professor Oliver Bandmann, and the Academy’s Dr Peter Fletcher, we designed the Research Engagement Meeting for professionals who are becoming or aspire to become involved with Parkinson’s research on many different levels.

The result was a stimulating meeting that started with defining the landscape of Parkinson’s research, showcasing how to move from a great research idea to delivering it in clinic, via giving an overview of how to get started in research and the support that is available whilst embracing the considerations from the perspectives of those living with the condition, and pragmatic approaches from those who have just started the journey.

In his introduction to the meeting CPT Trustee and Parkinson’s Academy Founding Faculty member Dr Peter Fletcher noted that we are currently at an interesting intersection in that there are trials of potentially disease modifying treatments underway and more on the horizon. The picture is changing rapidly and with urgency which makes this an exciting time to become involved in Parkinson’s research and importantly make that research available to patients.

Jo Johnson, consultant neuropsychologist and author of ‘Surviving Me’ presented a valuable webinar for professionals alongside Ruth Stross, MS Nurse Specialist. 

Presenting practical ideas and exercises for achieving and maintaining a good level of ‘mind health’, over thirty MS specialist nurses and similar practitioners attended the live webinar. 

Jo asserts that knowing your personal values makes you far more capable of managing a whole range of physical and mental challenges in times of crisis, and that actioning your values helps move you towards who you want to me. Essentially, aligning your values and who you are (your essence) and what you do (your form). 

Jo advocates considering the two ‘gears of the mind’, and notes that both gears are important, and both are needed in different scenarios. 
She highlights that ‘thinking gear’ is essential for thinking things through, planning, remembering, and such. However, by staying in it all of the time, that can lead to stress, anxiety and sleeplessness. Often our ‘thinking gear’ can take us away from our values and the actions we want to take to be more ourselves. 

‘Sensing gear’ is when we are in the present, experiencing our current situation, calling on our senses, and just ‘being’. When we are in ‘sensing gear’ we notice our thoughts, notice how we feel, and can make better choices about what we do as a result of them. Learning to get back into ‘sensing gear’ more often allows us to make choices that take us further towards the person we want to be, or actions that are more in line with our values.

Richard explains that the content of this webinar has come from experiences in holding remote clinics and acknowledged that many clinicians who have remained in their usual roles will probably have found their own way forward. He notes that those who had been redirected to frontline services and are now resuming some of their core roles with Parkinson’s patients again will likely benefit most. However, sharing examples of good practice across peers is often useful to all.

Some caveats for this webinar content included:
– This discussion is grounded in the context of COVID-19 and that these ideas are not a replacement for personal consultations but a means of meeting a need in the current pandemic. There may be learning from this which affects how we consult in the future, but there will always be a place for face to face clinics.
– ‘Remote consulting’ has quite negative connotations, so perhaps ‘virtual consulting’ is a better term. 
– Rather than telephone consultations which many have done previously, this is addressing the video-link element of the virtual consultation. 
– We are not discussing hospital to hospital video consultations, but clinical to patient consultations.
