In the first episode of the Program Spotlight podcast series, we speak with members of the palliative care team from CHI Health Palliative Care in Omaha, Nebraska. At the time of this recording, CHI provided inpatient consults across 5 hospitals, with a projected 1,300 consults this year — over 3,700 patient encounters. The program is supported within the clinic structure and works closely with the hospital medicine program. With two physicians, one nurse practitioner, 3.5 RNs, and administration support, pastoral care, and social services, the palliative care team is able to provide care across multiple campuses.

Getting Institutional Buy-In with CHI Health Palliative Care

Immune Checkpoint inhibitors have become an important treatment option in patients with metastatic lung cancer. This podcast gives an overview on the current evidence of immune checkpoint inhibitors in patients with non-small cell lung cancer. Based on the phase III data, patients with PDL1 expression >50% are candidates for 1st line pembrolizumab if there are no contraindications. Currently, PDL1 expression as assessed by immunohistochemistry is the most validated biomarker although other biomarkers including mutational load, neo-antigen presence or tumor infiltrating lymphocytes are currently under investigation. Importantly, the side effect profile of immune checkpoint inhibitors does not differ in lung cancer patients. Studies are currently investigating the value of immune checkpoint inhibitors for patients with small cell lung cancer. Several currently on-going studies are investigating the combination of chemo-/radiotherapy and immune checkpoint inhibitors in patients with lung cancer.

Immunotherapy in lung cancer

Michelle Meyer is an Assistant Professor and Associate Director of Research Ethics at Geisinger’s Center for Translational Bioethics and Health Care Policy in Danville PA. We spoke a few months ago about: her background; choosing to study both law and religious studies; having a doctorate in religious studies while being a nonbeliever; being a professional misfit; the weirdness of the institutional review board system; how we manage biospecimens and data; whether we should read social media terms of service agreements; why it’s not always a big deal when social media companies do research; what it’s like to live with an uncertain version of the gene responsible for Huntington’s disease; and what the public gets wrong about Huntington’s and other genetic diseases.

Julia Sweeney is an actress, comedian, writer and film director. She was a memorable cast member on Saturday Night Live from 1990 to 1994. After SNL she became most well-known for her monologues. Her first one-person show, God Said, Ha!, dealt with her brother’s and later her own cancer diagnosis and treatment. It played on Broadway, became a film, and was nominated for a Grammy. Subsequent monologues chronicled Julia’s quest to become a mother and adoption of a child as a single person, and her loss of faith and embrace of science. Her memoir, If It’s Not One Thing, It’s Your Mother, was published in 2013. Her new show, Older and Wider, debuts at Second City in Chicago on Friday January 12. We talked a few months ago about astrology, genetics, cancer, family, and her life as a Neanderthal churchgoing atheist.
