Eine Sonderform der Konversion ist das Affektäquivalent. Dabei sind seelische Reaktionen und die damit verbundenen körperlichen Erscheinungen voneinander abgekoppelt. Das bedeutet, der Patient empfindet nur die körperlichen Reaktionen. Auch hier liegen unterschiedliche Abwehrmechanismen zugrunde. Körperlich-vegetative Symptome wie Herzrasen, Schweißausbrüche oder Atemnot können beispielsweise Ausdruck von Angst sein. Sie werden daher als "larvierte Angstzustände" bezeichnet. Der Behandelnde muss also dem Patienten klar machen, dass seine körperlichen Symptome Ausdruck seelischer Konflikte sind, die wiederum aufgedeckt und verarbeitet werden müssen.
