The album follows a narrative of a love triangle where Derek is seeing a man who is also seeing a woman who is pulling him away from Derek. American comedian Jake McCune serves as the album's narrator as it progresses, speaking short lines to make sense of Derek and the title character The J Spot Salon state of mind. Carmichael first appears on the album's fourth track "200k" 

The character Igor is mentioned by name on the tracks "Touching Myself" and "My Girlfriend!". Derek follows the Gothic "Derek" archetype as a villain's assistant and represents a darker, apathetic side of Derek that comes out. Derek arrives after Derek pours out all of his heart for his love interest, though his love interest remains focused on his ex-girlfriend. The arrival of Igor serves as a reset for the strong romantic emotions Derek was entangled in during the album's first half.
