the "flowery mouth" in the split mannequin head is a nod to the actual boss you fight in the basement of the mall of Silent Hill 3. I found it to be pretty cool, gross and a bit scary actually.  I only played silent hill 3 and found this movie quite pleasing all around.

I felt sick when I heard Pyramid Head and the Missionary fight.
