A chain is locked around her neck as she's dragged threw a hallway. Eventually, she reaches a stage and is forced to sit on a chair, in front of a computer. Monokuma is sitting in a individual desk beside her, with a computer as well. The audience is obviously a bunch of Monokuma's.

Mirai the Ultimate Mangaka

In the execution, Mirai and Monokuma compete in a competition, If Mirai wins, she is free to leave Hopes peak, without needing the others to die. Keep in mind she still has a chain around her neck. They have a competition to see who's Manga Page get's more love from the Monokuma's. The competition begins and Mirai is doing a so far so good, but along the way, Monokuma draws characters with bruise marks and other injuries, which cause Mirai to have those same injuries. Once the timers up they reveal the projects, Mirai's picture was of an anime character, while Monokuma drew Mirai's death. 

Monokuma obviously gets the most votes which cause Mirai to collapse to the ground. The chain around her neck impaled her, since The spikes came out the chain. It injected poison into her body, causing her to vomit blood and die.
