The shoulder is a pretty complex joint, with many structures that can cause your patient's pain.

And it does not help that so many of your so called shoulder 'special' tests are not that special...

Most having poor sensitivity and specificity, meaning it can be hard for you to nail down a clear shoulder pain diagnosis.

So you might think, given that you do not have a clear diagnosis, there is not much you can do...

It is so easy to treat the wrong shoulder pain problem. 

You find a ACJ problem...

So you treat the ACJ.

You diagnose a rotator cuff, bursa or proximal bicep injury, so you focus your attention on that injury...

Even though your patient does not actually have a problem at that part of the shoulder!

WTF!!!

Why would you expect your patient to improve if you are treating the wrong thing?
