This version of the Fever theme (the "full" theme) is four bars longer than the version of Fever that is heard more often. The former version of Fever was first heard in the Game Boy version of Dr. Mario released in 1990 while the latter version was first heard in the NES counterpart of the same game.

The aforementioned four bars are present directly before the repeat (at about 1:04). 

This full theme appears in:
- Dr. Mario (1990, Game Boy)
- Tetris & Dr. Mario (1994, SNES; slightly modified)

...but does not appear in:
- Dr. Mario (1990, NES)
- VS. Dr. Mario (1990, arcade)
- Dr. Mario (1992, Gamewatch Boy)
- Dr. Mario 64 (2001, N64; eight bars longer than usual rather than four, but a different tune)
- Dr. Mario Online ℞ (2008, Wii)
- Dr. Mario Express (2008, DS)
- Dr. Luigi (2013, Wii U)
- Dr. Mario: Miracle Cure (2015, 3DS)
- Dr. Mario World (2019, mobile)

The full theme is also heard outside the Dr. Mario series, like in the Super Smash Bros. series.
