Intrapatient variation in PD-L1 expression and TMB and impact on ICI therapy in patients with NSCLC

In this Article of the Month podcast Annals of Oncology Editor-in-Chief discusses the ENGOT-en11/GOG-3053/KEYNOTE-B21 trial with Dr Toon Van Gorp of the Leuven Cancer Institute, Belgium.  This randomised study evaluated adjuvant pembrolizumab plus chemotherapy for newly diagnosed, high-risk endometrial cancer, showing that while adjuvant pembrolizumab plus chemotherapy did not improve DFS in the intention-to-treat population, it did improve DFS in patients with dMMR tumours, as shown in prespecified subgroup analyses.

ENGOT-en11/GOG-3053/KEYNOTE-B21: a phase III study of pembrolizumab or placebo + adj CHT +/- RT in patients with newly diagnosed, high-risk endometrial cancer

In this Article of the Month podcast Annals of Oncology Editor-in-Chief Tom Powles discusses a first-in-human study of AMG 193 with Dr Jordi Rodon, MD Anderson Cancer Center, Houston, USA. AMG 193 is a first-in-class, novel, MTA-cooperative PRMT5 inhibitor and a highly selective small molecule designed to specifically target MTAP-deleted cancer. In vitro, AMG 193 preferentially inhibited the MTA-bound PRMT5 enzyme in MTAP-null tumor cells. In vivo, AMG 193 inhibited the growth of multiple MTAP-deleted tumor xenograft models.

Avelumab+axitinib vs. sunitinib as 1st line for advanced RCC: the phase III JAVELIN Renal 101 trial

In this Article of the Month podcast, Annals of Oncology Editor-in-Chief Tom Powles and Professor Jenny Seligmann of Division of Oncology, Leeds Institute of Medical Research, University of Leeds, Leeds, UK, discuss the FOxTROT trial.  This is the first randomised trial to test a neoadjuvant-targeted agent in biomarker-selected locally advanced colon cancer.  The study found that In RAS/BRAF-wt patients, addition of panitumumab to neoadjuvant FOLFOX showed a trend towards reduction in cancer recurrences.  The panitumumab effect was more marked in biomarker-hyperselected patients, and for DFS and OS.

In this Article of the Month podcast Annals of Oncology Editor-in-Chief Tom Powles discusses  the recently published study on WGS-powered ctDNA sequencing for breast cancer detection, with Prof. Nick Turner, The Institute of Cancer Research, London.  WGS-powered ctDNA detection improved detection at baseline and follow-up and increased lead time to clinical relapse.  ctDNA detection during follow-up strongly associated with worse relapse-free and overall survival.  Ultrasensitive assay identified that a minority of early MRD+ patients clear ctDNA and do not relapse during long follow-up.
