Medicare is a vital health insurance program for older adults, but it also serves individuals under the age of 65 who meet certain disability criteria. While many people associate Medicare with those aged 65 and older, it is equally important for those under 65 who have qualifying disabilities. Understanding how Medicare eligibility works for disabled individuals is key to ensuring they receive the necessary health coverage. This article outlines the Medicare eligibility rules for disabled individuals under the age of 65.

Eligibility Criteria for Disabled Individuals

To qualify for Medicare as a disabled person under 65, certain conditions must be met. Unlike those aged 65 or older who are generally eligible based on age, eligibility for individuals under 65 is based on a disability determination.

1. Disability Status

To be eligible for Medicare under 65, an individual must have a disability recognized by the Social Security Administration (SSA). This typically means the person has a condition that severely limits their ability to work and is expected to last at least one year or result in death. Common disabilities that can qualify someone for Medicare include conditions such as heart disease, certain types of cancer, severe mental health disorders, neurological conditions, and mobility impairments.

2. Receiving Social Security Disability Insurance (SSDI)

For a person with a disability to qualify for Medicare, they must be receiving Social Security Disability Insurance (SSDI) benefits. SSDI is a program for individuals who have worked and paid Social Security taxes for a sufficient number of years but can no longer work due to a disability.

Once an individual has been receiving SSDI benefits for 24 consecutive months, they automatically qualify for Medicare. The 24-month waiting period starts the first month the person starts receiving SSDI benefits.

However, for certain conditions, such as end-stage renal disease (ESRD) and amyotrophic lateral sclerosis (ALS), individuals can qualify for Medicare without having to wait the 24 months. These individuals are eligible for Medicare as soon as they begin receiving SSDI benefits, bypassing the typical waiting period.

Medicare Coverage for Disabled Individuals

Once eligible, individuals under 65 will receive Medicare Part A (hospital insurance) and Medicare Part B (medical insurance). These two parts form the foundation of Original Medicare, which covers essential health services.

Part A covers inpatient care, skilled nursing facility services, hospice care, and certain home health services. Most people are eligible for Part A without paying a monthly premium if they or their spouse have worked and paid Medicare taxes for at least 10 years.


Part B covers outpatient care, doctor visits, preventive services, and some medical equipment. Unlike Part A, Part B requires a monthly premium.


Additional Coverage Options

Medicare, though comprehensive, does not cover everything. Disabled individuals under 65 who are enrolled in Medicare may also choose to enroll in additional coverage to reduce out-of-pocket costs:

Medicare Advantage (Part C): This plan is offered by private insurance companies and combines the benefits of Part A and Part B, often with added coverage for services such as vision, dental, and hearing. Some plans also include prescription drug coverage (Part D).


Prescription Drug Coverage (Part D): Part D helps cover the cost of prescription medications. It is available through private insurance companies and can be added to Original Medicare if not already included in a Medicare Advantage plan.


Medigap (Medicare Supplement Insurance): Medigap policies help pay for some of the out-of-pocket costs not covered by Original Medicare, such as co-payments, coinsurance, and deductibles.

Medicare Eligibility For Disabled Individuals Under 65

Abby Fines is an Assistant Professor in the Department of Kinesiology and the Director of the Kinesiology Lifetime Physical Activity (KLPA) program at the School of Education and Human Development at the University of Virginia. She oversees more than 35 academic courses designed to equip students with the knowledge and skills needed to lead physically active lives. With a specialization in adaptive physical activity, Fines focuses on sports development for individuals with intellectual, physical, and sensory disabilities across various levels of play and competition. Her research is dedicated to creating equitable opportunities for participation, particularly for para-athletes, in both school and community athletic settings.
