The consequences of income inequality extend far beyond economic disparities. Stephen Bezruchka focuses on its effect on health. Stephen is on the faculty of the School of Public Health at University of Washington and is author of Inequality Kills Us All. Much of this interview looks at these issues in the US.

Stephen directly ties inequality to mortality rates, disease, and early childhood development. It’s interesting to hear a physician who doesn’t blame our individual choices and habits; he lays our health problems squarely at the feet of the political and economic system.

The profit-driven nature of healthcare in the US prioritizes financial gain over the health of individuals. Stephen describes the role of the lobbying industry and private corporations contribute to the perpetuation of this system.

The conversation concludes with a discussion on the concept of social murder and structural violence.

Stephen Bezruchka (pronounced bez rootch ka ) is faculty in the School of Public Health at the University of Washington in Seattle. He worked as an emergency physician for 30 years and also set up a teaching hospital in a remote district in Nepal where he supervised the training of Nepali doctors. His current work is in making better known what produces health in a population and why the United States has worse health outcomes than some 50 other nations despite spending almost half of the world's healthcare bill.

“But fortunately for us, human nature is precisely the capacity to be creative; to imagine a different way of living together and to bring that into being through our conscious and deliberate actions by working together, by fighting for a different world against the class power that is interested in perpetuating things as they are. And so we can, through acting on the world to transform the world, we also transform ourselves. That's a basic principle of Marxist theory and Marxist politics. By acting on the world to change the world, we simultaneously transform ourselves through the act of struggle, to the act of building collective solidarity, we become different and we also make the world different.”

Steve invited Stephen Maher for this interview to talk about some of the basic lessons of Marxism. While you may not agree with everything you hear in this episode, certain fundamentals of capitalism are beyond refute.

The discussion explores the relationship between capital and the working class, and the concept of class struggle as the key to understanding US history of the past century, especially the postwar period and the development of neoliberalism. To truly make sense of it all we must look at some fundamental truths about capital. It is very fluid and dynamic. Capital is capable of continuously evolving and restructuring. In doing so, our social conditions change as well.

They also discuss the challenges and obstacles in achieving socialism, the history of anti-communist sentiment in the US, the importance of class struggle unionism, and the need for grassroots organizing and building solidarity within the working class.

Stephen Maher is an Assistant Professor of Economics at SUNY Cortland, and co-editor of The Socialist Register. He is the co-author of The Fall and Rise of American Finance: From J.P. Morgan to BlackRock with Scott Aquanno, and the author of Corporate Capitalism and the Integral State: General Electric and a Century of American Power.

Is Marx Still Relevant? with Steve Maher

A hundred years ago, John Maynard Keynes predicted we would see a massive expansion of growth and productivity under capitalism. Bingo. He got that right. He said this amazing efficiency would result in a shorter work week with far more leisure time for everyone. Oops. Not so much. The structure of capital ownership will never permit it.

Bill Mitchell’s blog post, Keynes Was Wrong Because He Failed to Consider Class Conflict, was the inspiration for this episode. Bill talks with Steve about many of the truths about capitalism economists and historians get wrong, beginning with the idea that capitalism freed workers from the bonds of feudalism.

Bill covers the social democratic politics of the postwar era and how the ruling elites then ensured that government would only serve as their agent. He and Steve take a critical look at the dangerous power of institutions like the World Bank and IMF. Bill explains why he supports degrowth instead of “green growth.”

Bill believes progressives shouldn’t be working to reform the system; they must work to change it. Coming from the Marxian tradition, he says MMT is only part of the story.

“Yes, it provides a first class lens into monetary operations but it doesn't have a theory of power. And you have to add that ideological layer for it to be, in my view, a purposeful framework for advancing systemic change. Just to have an MMT understanding provides you with no additional tools to work out systemic change.”

Class, Capitalism, and MMT with Bill Mitchell

At Macro N Cheese we’ve been having an ongoing conversation about the need for radical change in the labor movement and the rise of class struggle unionism.  

Traven Leyshon is president of the Green Mountain Central Labour Council and part of the leadership team of the Vermont State Labour Council. Traven, who has over five decades of labor organizing experience, talks to Steve about the pushback from national labor institutions against Vermont's newly elected progressive leadership and what it represents for the movement. 

The episode delves into the historical context of labor shrinkage and addresses the practical challenges of organizing in a neoliberal economy dominated by corporate money. He emphasizes the need for a participatory democracy within unions, contrasting this approach with the dominant model of business unionism, where members often feel disconnected and disengaged. 

Traven and Steve highlight the need for workers to have a deeper knowledge of how the system operates in order to fight back against the capitalist order. They also touch on issues such as the need for a job guarantee and the impact of austerity measures on working-class people.  

In the face of climate crisis and continual economic crises, building a powerful, inclusive labor movement is more urgent than ever.

715 — After the election organise the left: insist on Labour's pledges; Fight to restore NHS

"Cielos de Oportunidad" is a song that resonates deeply with me, both as a musician and as someone who reflects on the disparities in society. Initially conceived as an instrumental piece, this Delta Blues track has found new life with lyrics that poignantly address class struggles. Performed with a slide on a Cigar Box Guitar, the raw, earthy tones of the instrument perfectly complement the song's gritty thematic content.

The lyrics of "Cielos de Oportunidad" paint a vivid picture of a person's struggle through the early hours of a harsh morning, navigating life with resilience despite the cold and discomfort. The song begins with an evocative description of setting out in the freezing dawn, hinting at the daily grind of those who live on the margins of society. The use of a makeshift cart and the search for basic necessities like cardboard and food encapsulates a narrative of survival and perseverance.

Musically, the slide guitar's melancholic wails and the haunting melodies evoke the emotional landscape of the protagonist. The blend of traditional Delta Blues with a poignant narrative adds a powerful layer of social commentary, making "Cielos de Oportunidad" not just a musical piece but a profound statement on the human condition.

The chorus, with its catchy and repetitive phrases, underscores the hope and relentless spirit of the protagonist. Phrases like "¡Ah, ah! Cada cielo trae una luz, ¡si, si!" become anthemic, offering a stark contrast to the verses that detail the struggles. This dynamic between the verses and the chorus highlights the cyclical nature of hope and hardship.

As "Cielos de Oportunidad" progresses, it builds a narrative arc that moves from despair to a cautious optimism, mirrored by the gradual brightening of the day in the lyrics. The song's title itself, which I chose not to translate, stands as a testament to the overarching theme of finding opportunities in every new dawn, despite the adversities faced the night before.

"Cielos de Oportunidad" is more than just a song; it's an experience and a reflection on society's stark realities wrapped in the soulful strains of Delta Blues. It's a reminder of the power of music to convey deep social messages and evoke empathy and understanding in its listeners.

725 — Trump win boosts far-right: Back civil resistance in US; Unite the left for socialist policies

💿 Lost Dubs 005 – Freedom Will Come (2006)

2006 was a year I was digging deep into dub — not just as a sound, but as a philosophy. Freedom Will Come samples the words of an activist dub poet, echoing with the urgency of struggle and the promise of liberation.

“You can only push a dog around so many times before that dog will turn around and bite you.”

This wasn’t about the polished club floor. It was about the other side of dance music — the protest, the poetry, the call for change. Heavy bass, rolling percussion, and the voice of resistance carried through delay and reverb until it felt like a mantra: freedom will come.

Lost Dubs 005 - Freedom Will Come (2006)

The album closer reimagines the classic 1912 Lawrence Textile Strike song "Bread and Roses" for 2025, honoring the tradition while updating for intersectional struggles. WikipediaWMNF Opens with solo acoustic guitar and voice, traditional folk style, singing the original verse about hearts starving as bodies starve, about beauty and art being necessities not luxuries. "Hearts starve as well as bodies, give us bread but give us roses."
But then each verse adds a contemporary struggle: Trans people demanding not just survival but joy and celebration (gender splendor for everyone); disabled people demanding not just accessibility but full participation in society; reproductive justice meaning not just legal abortion but comprehensive care, childcare, paid leave; workers demanding not just living wages but dignity, democracy, and control over their labor; housing justice meaning not just shelter but community, culture, and homes that don't displace; immigrants demanding not just legal status but belonging and freedom from persecution.
Each verse builds instrumentally—first bass, then drums, then electric guitar—until the final chorus is a massive, soaring anthem with all the themes united: "We're fighting for our bread, our roses too / Trans liberation and reproductive rights / Labor unions and a place to live / Disability justice, immigrant freedom / All of it together, all of us alive / This is our time, this is our fight."

Bread and Roses (2025)

Based on the Daimler Truck workers chanting "tick tock" as their contract deadline approached, and the Boeing strike of 33,000 machinists who voted down the first contract offer by 95%. labornotes Heavy, mechanical beat mimics factory sounds. Verses tell stories of workers forced to work weekends despite record profits, automation threatening jobs, executives making 300 times what workers earn. Chorus is the relentless "tick tock" chant building in intensity. References the ILA dockworkers' first strike since 1977, the 53-day Boeing action, AT&T's 36-day strike. Bridge features actual union slogans: "On strike, shut 'em down, this is a union town!" Celebrates the 271,500 workers who struck in 2024 Economic Policy Institute and the 77% of young workers who approve of unions.

Tick-Tock, Tick-Tock

Takes its title from actual 2024-2025 housing justice slogans. Opens with the April 2025 Housing NOW! protests where activists wrapped city halls in red tape. National Coalition for the Homeless Verses expose private equity landlords: Greystar buying 8,500 apartments and jacking up rent 36% while cutting security and maintenance; ProPublica Blackstone raising rents 31.5% in Phoenix; Private Equity Stakeholder Project Cleveland where private equity purchases jumped from 7.2% to 21.1%. Lincoln Institute of Land Policy Contrasts Spain's hundreds of thousands marching for housing rights Al Jazeera with America's reality where the top 10% own 67% of wealth while the bottom 50% own 2.5%.
Chorus: "Housing over handcuffs, homes over greed / It's a human right, not a luxury / From DC to Nashville, San Francisco Bay / They're pricing us out, but we're here to stay." References the Poor People's Campaign organizing 140 million poor and low-income people. Poor People's Campaign Bridge is spoken-word from a tenant forced out: "I felt like I was forced out the door." ProPublica Ends with chant: "Whose city? OUR city!"

Housing Over Handcuffs

Raging against wealth inequality. Opens with the statistic: The world is on track for 5 trillionaires within a decade while 733 million people lack sufficient calories globally. Verses detail the obscenity: Billionaire wealth grew $2 trillion in 2024 ($5.7 billion per day), Wall Street bonuses hit $47.5 billion, the top 10% own 67% of wealth while the bottom 50% own 2.5%. Oxfam GBWorld Socialist Web Site Meanwhile, 13.4% of children live in poverty, child poverty more than doubled after the expanded Child Tax Credit expired, Black children experience poverty at three times the rate of white children. Firstfocus
Heavy, aggressive rap verses with screamed chorus: "FIVE TRILLIONAIRES! While we can't pay rent! / FIVE TRILLIONAIRES! Our wages ain't been spent! / From the bottom to the top, there's a class war / And they're winning it while we're dying poor!" References the June 2024 Kenya protests where youth rose up against tax bills targeting the poorest; Bangladesh where 400+ students were killed protesting job quotas. Human Rights Watch Bridge lists the names: Bezos, Musk, Zuckerberg—and asks "What do we do with the enemies of humanity?" Leaves the question hanging ominously.

Five Trillionaires

A celebration of young workers leading the labor resurgence. Opens with the statistics: 77% of workers under 34 approve of unions, 88% of workers under 30, support crossing partisan lines. Center for American ProgressTraprock Verses tell the stories: The predominantly young Starbucks Workers United organizing 537 stores; Restaurant Dive grad students organizing 20,000+ since 2022; young workers at Amazon, in tech, in schools bringing "imagination, creativity and fresh strategies" to the movement.
Chorus is defiant: "We're not asking, we're organizing / No one's coming to save us anyway / The adults haven't won, only we can / Gen Z ain't asking, we're here to stay." References actual youth organizing language and approach. Bridge addresses why: Starting from behind, crushing student debt, housing unaffordable, climate crisis, no job security, nothing left to lose. But turns that precarity into power: "You gave us nothing, so we've got nothing to lose / You broke the system, now we're breaking through." Ends with union chants: "I believe that we will win!"

Gen Z Ain't Asking

Opens the album with the GLAAD statistic: 932 anti-LGBTQ incidents in one year, 2.5 per day. But the song expands to show this is part of a larger pattern—575 anti-LGBTQ bills, aclu 27 states banning gender-affirming care, aclu 13.4% of children in poverty, Firstfocus 271,500 workers on strike. The chorus chants: "An injury to one is an injury to all / They're coming for us, they're coming for all / No left and right, just have and have not / 932 days till we tear this whole thing apart." References the Gender Liberation March linking trans rights and abortion rights, the intersectionality between all struggles. The 19th News Sets the urgent, angry, but hopeful tone.

932 Days
