THCPO (Tetrahydrocannabiphorol-O-Acetat) ist ein synthetisches Cannabinoid, das 2021 erstmals entwickelt wurde. Es gehört zur Gruppe der acetylierten Cannabinoide, was bedeutet, dass es chemisch modifiziert wurde, indem eine Acetatgruppe angehängt wurde. Diese Modifikation beeinflusst die pharmakologischen und toxikologischen Eigenschaften erheblich. THCPO ist in Deutschland seit Juni 2024 verboten, wird jedoch in einigen Nachbarländern noch legal oder in rechtlichen Grauzonen angeboten.

THCPO wirkt stark psychoaktiv und wird als extrem potent eingeschätzt, ähnlich wie synthetische Cannabinoide in Spice-Produkten. Seine Wirkung basiert auf einer starken Bindungsaffinität zum CB1-Rezeptor, mit einer potenziell 30-fach höheren Potenz im Vergleich zu THC. Erfahrungsberichte beschreiben eine lang anhaltende euphorische Wirkung, jedoch können Nebenwirkungen wie Paranoia und Kreislaufprobleme auftreten. Medizinische Anwendungen von THCPO sind bisher nicht erforscht, und es fehlen Daten zu Langzeitfolgen.

Ein großes gesundheitliches Risiko besteht beim Verbrennen von THCPO, da das giftige Gas Keten freigesetzt wird, das bereits in kleinen Mengen toxisch ist. Keten entsteht bei Temperaturen über 340°C, die beim Rauchen leicht erreicht werden. Aufgrund fehlender Qualitätskontrollen in Grauzonenprodukten kann die genaue Menge des freigesetzten Ketens nicht bestimmt werden.

Ob THCPO bei Drogentests nachweisbar ist, ist noch unklar, jedoch könnte es aufgrund seiner chemischen Ähnlichkeit zu THC zu falsch-positiven Testergebnissen kommen.
