I'd write a proper description but this is probably self explanatory.
Phil (aka Philza) is now the Frisk counterpart, he takes a lot after his alternative 'winter' skin in the dream smp so it's not just literally Kisuke from Bleach.

And Martyn now has a physical appearance; he follows the player continuously as a very small crow either flying around or on Phil's shoulder.

Beds of various colors take the role of SAVE points. This is the very first iteration of the Start Menu theme which lasts up til you befriend Lizzie (there's no Judge Judy & Executioner iteration).

Also it shows Phil (or whoever you name your character)'s name in the beds and save menu now instead of Martyn's, but Martyn's name still shows up in battle.

Since Philza is the new Frisk replacement...the Hardmode replacement for this AU is "Hardcore Mode", which makes all of the bosses and enemy encounters much more difficult, and gives you only one try akin to a roguelike; fail once and back to the beginning with you.

the sans jacket is entirely a coincidence
yes this is gaster here, hes pretty much the same in this au, except he wasnt actually scattered across time and space, hes just...y'know, dead (though his presence still lingers around, and there's a room with him in it)

aleyna is the name of chara counterpart, the frisk replacement is called Rin and she is CRACKED despite her appearance, wields both a katana and handgun and can use either in combat (as well as in several cutscenes)

Years after WW3, 1 great nation ruled over the ruins of what was called EARTH.

The nation leaders of the nation promised to ensure that peace would be maintained for eternity.

However, this promise was broken as power went to their heads, and soon their dictatorship grasped the nation with an iron fist.

 A REBELLION formed and yet again, another great war broke out between the 2 sides. The uprising, and those hungry for power.

In the end, the power hungry reigned victory, and trapped the rebellion inside of a small district with a magic spell, ensuring they’d never be able to threaten their statuses again.

YEARS LATER - THE COVE DISTRICT

Years have passed, but the rebellion continues inside of the magical walls they’ve been trapped in.

And now it’s your job to finish what your nation has started.
