Twinkle Twinkle Little Star! For Kids!

This idea came about during a chat with Jason64.

This IT is located in front of the Barrier in New Home after you separate the Mettaton IT. The IT looks heavily similar to a Reaper Leviathan from Subnautica, but made out of several dead bodies morbidly stitched together, suspended from the ceiling by several meat hooks and has its mouth gagged. Once the gag has been removed, it will start screaming and damage the player 1 HP per turn. It becomes separable via removing the gag on its mouth and calming it down.
