Podcast from the NHS England and NHS Improvement National Patient Safety Team, where Tracey Herlihey, head of patient safety incident response policy, and Lauren Mosley, head of patient safety implementation, talk about the Patient Safety Incident Response Framework (PSIRF) which will be launched in Spring 2022. The framework is a key component of the NHS Patient Safety Strategy, and will outline how NHS providers should respond to patient safety incidents and how and when a patient safety investigation should be conducted. Once implementation is completed it will replace the current Serious Incident Framework.
 
The podcast gives an overview of PSIRF and its key features, talks about findings from work with early adopters over the past two years to pilot an introductory version of the framework, and explains what providers can do now to prepare for its launch in the Spring.

Engaging and involving patients, families and staff following a patient safety incident

This podcast focuses on the training NHS organisations should ensure staff undertake as part of their preparation for the implementation of the Patient Safety Incident Response Framework (PSIRF) by Autumn 2023. The podcast provides an introduction to the training requirements, the PSIRF training procurement framework, training offered by the Healthcare Safety Investigation Branch, and an overview of how this fits in with the wider national patient safety syllabus. PSIRF support – expert tips on communicating large scale change

The leadership and management functions of the Patient Safety Incident Response Framework (PSIRF) oversight are wider and more multifaceted compared to previous patient safety response approaches. In this latest podcast we are joined by Lauren Morgan, director of Morgan Human Factors Ltd, and Donna Forsyth, director of Patient Safety Science not for profit Ltd, to discuss the PSIRF ‘oversight roles and responsibilities specification’ document. PSIRF oversight: a shift towards engagement and empowerment

As part of preparation to transition to the Patient Safety Incident Response Framework (PSIRF), organisations are required to create a patient safety incident response policy and plan. 

In this podcast, we are joined by Jacqui Fletcher, Senior Clinical Advisor for Stop the Pressure Programme and National Wound Care Strategy, who we collaborated with to develop a guide to applying PSIRF principles in relation to planning how to respond to pressure ulcers. Considering pressure ulcers in PSIRF planning

In this podcast, GPs and medical examiners from England and Wales discuss their experience of extending medical examiner scrutiny to deaths in community settings. It explains how partnership working between GPS and medical examiners improves the experience of bereaved people and explores advantages for GPs of starting work with medical examiners now, before medical examiners become a required part of the new statutory death certification process from April 2024.

Medical examiners are senior doctors from all specialties including GPs, who provide independent scrutiny of deaths not taken for investigation by coroners, and offer the bereaved an opportunity to ask questions and raise concerns.

GPs and medical examiners working together

This podcast focuses on the vital role of NHS urgent community response services and how they are helping people stay well and independent at home. Bringing together patient safety, QI and organisation development for system transformation

We recorded this podcasts as part of our work to mark World Patient Safety Day 2023, to celebrate its theme of ‘engaging patients for patient safety’. How a household model helps GPs work in partnership with people and communities

Working in partnership with people and communities to support stroke patients in West Yorkshire

Working in partnership with people and communities to improve support for diabetes in Nottingham

The Patient Safety Incident Response Framework (PSIRF) is a new approach to learning from patient safety events in a way that leads to improvement in care. It was published in August 2022 and mandated in the NHS Standard Contract in April 2024.     

In this podcast, Tracey Herlihey from NHS England’s national patient safety team talks with Jenny Coverley from Middlewood Practice in Cheshire about her experience applying PSIRF principles in general practice.

Applying PSIRF in general practice

Your GP record suggests you may not have had 2 doses of the MMR vaccine. You need both doses for long-term protection against measles, mumps and rubella. 

It's now quicker and easier for you to catch up. You can book an appointment with your GP surgery, or you may be able to get your free MMR vaccine at some community pharmacies in certain areas. You can check if pharmacies are offering MMR vaccine walk-ins in your local area using the links at the top of this web page.  

You do not need to do anything if you’ve already had 2 doses. Please check your vaccination history in the NHS App, or you can contact your GP surgery if you’re not sure. 

The NHS offers MMR vaccination because these infections spread easily between people. They can lead to serious problems including meningitis, blindness, hearing loss and risks during pregnancy. It’s important you get fully vaccinated because measles cases are increasing in England. MMR vaccination is the most effective way to protect yourself. It also helps protect people who cannot be vaccinated, such as unborn babies, newborn babies and anyone with a weakened immune system.
