Trauma, enkele beschouwingen over trauma
Wat is een trauma nu eigenlijk? Trauma’s horen bij het leven maar toch lijkt het alsof er ondertussen een inflatie is van wat nu een problematisch trauma is. Trauma lijkt eigenlijk vooral heel veel te maken hebben met de context waarin het plaatsvindt. Wat is de betekenis voor degene die het overkomt. Daarbij is dus minder belangrijk wat nu echt het mentale of fysieke trauma is. Als sociale dieren hebben ingeboren drempel om elkaar echt kwaad te doen. Toch gebeurt het: oorlogen zijn van alle tijden en daar zit een interessante dynamiek achter. Hoe je als je als soldaat gedood hebt, hoe je daar weer daarmee met jezelf in het reine kan komen. Dit heeft opnieuw alles te maken met de context waar we net over hadden. Zowel dus bij daders als bij slachtoffers. Wat betreft deze context bespreken we het gegeven dat ieder mens een aantal illusies over eerlijkheid en veiligheid in het leven heeft. En dat deze toch ook geschonden kunnen worden. We bespreken dat deze context cultuur en tijd gebonden is. 100 jaar geleden dacht men er totaal anders over wat nu eigenlijk een trauma was. De weging van een gepresenteerd trauma, en dus zeker ook van de context, is een belangrijke taak van de hulpverlener. Zo wordt een door mensen veroorzaakt, en zeker op opzettelijke wijze, trauma veel ernstiger ervaren dan bijvoorbeeld een trauma door natuurkrachten. We bespreken ook het fenomeen weerbaarheid in deze context: trauma wordt altijd ernstige beleefd wanneer men zich weerloos voelt. Dus ook je reactie op een trauma is relevant in de beleving ervan. Wanneer je het gevoel hebt krachtig te zijn en de weerbaar en daar mogelijk ook naar gehandeld hebt en wordt een trauma aanzienlijk beter verwerkt. Dit maakt ook dat wraak, door bijvoorbeeld van aangifte, zinvol is in traumaverwerking; dit wordt helaas nogal eens vergeten.

Aflevering 17 - Trauma

Bipolaire stoornissen
De bipolaire stoornis kenden we vroeger als de manisch depressieve stoornis. Een stemmingsstoornis die aangeboren is, tussen 25e en 45e jaar tot uiting komt en soms heftig uit de hand kan lopen. Zo erg zelfs dat iemand de realiteit uit het oog verliest en psychotisch wordt. We bespreken de eigenschappen van de depressie en de manie en ook hoe deze onderscheiden kan worden van de persoonlijkheidsstoornis die ook veel stemmingsstoornissen geeft: de borderline stoornis. Tenslotte welke adviezen we geven, welke medicatie zinvol, is en wanneer we soms zelfs gedwongen moeten ingrijpen.

Aflevering 18 - Bipolaire Stoornissen

Angststoornissen 
Angst is een normaal gevoel dat de mensen beschermt voor het doen van domme en gevaarlijke dingen. Soms slaat het apparaat wat angst registreert in ons hoofd op hol en dan kunnen spreken van een angststoornis. Er zijn twee categorieën: irreële angst, dus angst voor dingen waar je helemaal niet bang voor hoeft te zijn, zoals bijvoorbeeld de paniekstoornis, de specifieke fobie en de sociale fobie en de obsessief compulsieve stoornis. En wat we daar eens wat aan moeten doen: vermijding doorbreken. Zo komen we bijvoorbeeld bij exposure behandelingen: juist het weer aangaan van wat juist zo eng lijkt te zijn. En dan ervaar je dat die angst zomaar kan komen maar ook zomaar weer kan weggaan. De andere categorie is wanneer bij een daadwerkelijk ernstig incident de angst blijft hangen. In dat geval spreken van de posttraumatische stressstoornis (PTSS). In een latere aflevering gaan we uitgebreider hierop in.
Tenslotte hebben we nog de gegeneraliseerde angststoornis die ook voor deze psychiater een wat onduidelijke categorie is.

De Biografie; hoe neem je die af en waarom eigenlijk?
Deze aflevering staat een van de pilaren van het vak psychiatrie aan bod. Immers het beschrijft de levensloop van de client. Helaas is in de moderne tijd hier veel te weinig aandacht voor. Immers we willen graag denken dat het meest complexe orgaan van het lichaam, het brein, met richtlijnen en modules te behandelen is. Welk een misvatting! Een goede biografie geeft daadwerkelijk inzicht in de mens achter de klacht en maakt het werk zo veel meer boeiend. Daarnaast behoedt een goede biografie ons voor kortzichtig handelen zoals boven beschreven. En is daarom ook, in tegenstelling wat men denkt ook nog kosten efficiënt. Verder beschrijven we in een kort bestek hoe je snel een biografie afneemt.
