INTRODUCTION
Around the world, prices of a wide range of goods and commodities have been rising sharply for the past few months. In particular, the wholesale cost of energy has been rising fast as the world economy recovered from the pandemic restrictions. Petrol prices have risen by almost a third in the past 12 months. The UK domestic energy ‘price cap’, which hit a low of £1,042 in 2020, is expected to rise to £2,800 in October. Consumer price inflation has hit 9% and is likely to reach 10% by the end of the year. For those on lower incomes, who spend more of their income on food and energy, the impact is even greater.

There are multiple explanations for the rises: the post-pandemic recovery and problems with shipping have been widely cited. The war in Ukraine and sanctions against Russia are hitting energy and food prices. Many economists also point to the rise in the money supply – thanks to ultra-low interest rates, quantitative easing and huge government spending programmes. Rises in production have not kept pace with rising demand, so prices have risen.

But the other side of the story is that wages are not keeping up with rising prices. As a result, most people are seeing real-terms cuts in their living standards. Governments and central bankers seem desperate to keep a lid on wage rises, desperate to avoid a ‘wage-price spiral’, but the effect is to make most people significantly poorer. Those on fixed incomes may be hardest hit of all.

What are the main reasons for the rise in living costs? What can be done to help reduce the impact? Should we be looking beyond short-term and temporary factors? Is this a crisis that has been coming for some time?

Jenny Mueller, MD, is a gynecologic oncologist and assistant attending in the department of surgery at Memorial Sloan Kettering Cancer Center. She leads the endometrial cancer research team at MSKCC with an emphasis on prospective, translational, and collaborative efforts within and across institutions. Bill Zammarrelli, MD, currently works as a gynecologic oncology fellow at Memorial Sloan Kettering Cancer Center. He is a commissioned officer in the US Army and completed his residency at Walter Reed National Military Medical Center. His current research focuses on the genetics of endometrial cancer. 

Highlights:

- PORTEC-1 and GOG-99 risk classifications are discordant for stage I grade 3 endometrioid endometrial carcinoma (EEC).

- Stage I grade 3 EECs of CN-high molecular subtype have a worse 3-year progression-free survival compared to non-CN-high molecular subtypes.

- Molecular classification in combination with clinicopathologic factors may provide improved prognostic information.

Percutaneous management of acute ischaemic stroke

In this episode of the IJGC podcast, Editor-in-Chief, Dr. Pedro Ramirez, is joined by Dr. David Cibula to discuss the 2023 ESGO-ESTRO-ESP Guidelines for Cervical Cancer. Dr. Cibula is the Chair of the Department of Obstetrics and Gynecology at the General Faculty Hospital in Prague, Professor at the First Medical Faculty of the Charles University, Chair of the Central and Eastern European Gynaecologic Oncology Group (CEEGOG), member of the Strategic group of the ENGOT (European Network for Gynaecological Oncological Trial Groups) as well as a Former ESGO president.

 Highlights:

- Multidisciplinary clinical practice guidelines developed jointly by ESGO/ESTRO/ESP in the field of gynaecological oncology, radiation oncology and pathology.

- Guidelines cover the management of all stages of cervical cancer, including separate sections dedicated to pathology reporting and management in pregnancy.

- The guidelines include recommendations, algorithms, and summaries of recent evidence published as supplements.

ESGO-ESTRO-ESP Guidelines for Cervical Cancer 2023 with David Cibula

In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Rene Pareja to discuss recent the cervical cancer management updates. Dr. Pareja is a gynecologist-oncologist at Astorga Oncology Clinic in Medellín and the National Cancer Institute in Bogotá, Colombia. He is a reviewer for more than 20 specialty journals and is an Associate Editor for IJGC. He is the author of ten book chapters and more than 80 publications in peer-reviewed journals, and at IGCS 2021 he received an award for Community Advancement in Resource-Limited Settings.

Highlights:
 - There is controversy in offering minimally invasive approaches to women with early-stage cervical cancer and tumors less than 2 cm.
 - The role of surgical staging remains controversial because guidelines still recommend it, despite evidence of a negative prospective randomized controlled trial.
 - Intermediate risk factors after a radical hysterectomy need to be re-evaluated in a very well designed prospective randomized trial, with modern radiotherapy techniques and updated inclusion criteria.
